I will admit that this one is a bit hit and miss and I am flagging it more as I want to go to bed and I know I will end forgetting in the morning.
I haven't found the trigger exactly but it has happened with two separate saves on the third scenario, it occurs when either the Seraphim uses Awakening, the Paladin uses Drawfire or the Blackjack using Awakening. I will update this post when I fully identify the trigger but just in case this is one of those cases where you can work with log alone, I shall leave it below.
